Records requested:

The following documents are requested for calendar years 2021 and 2022 to date:

Documents identifying any elected official who was covered by health insurance paid for (in whole or in part) by the Township of Bridgewater for any portion of the year, and for each such person, documents indicating the following:
1. The amount paid by the Township of Bridgewater for the health insurance during that year, net of any contribution by the elected official;
2. The amount contributed by the elected official for the health insurance during that year;
3. The type of coverage, e.g. individual, family or couple;
4. The services covered, e.g. medical, prescription, dental, vision, etc.

Notes:

A. This request is for information pertaining to elected officials only.
B. This request seeks information solely pertaining to insurance coverage and does not seek health or medical information for any person.
