Attached are the Health Department records for 1262 Bear Tavern Road,
Block 62, Lot 4.01. These are the Health Department records only, further
documents may be coming depending on the scope of the request.
If this inquiry is in anticipation of an upcoming real estate transfer,

the Fire Safety and the Health Department. Failure to file applications
