Dear Brick Township,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Records requested:

1) May I please have the enrollment benefits guide/summary that would include all of the plan summaries offered and the benefit rates for all plans for staff, ADMIN and Non-Represented for 2023/2024 and 2024/2025. This will be for Health, Dental and RX. Please also include vision if you offer.

2) Broker Presentations for Health, RX and Dental given to the district from your health broker. At this point I do not know who it is, but I will put in a request next for the contract under Immediate Access.

3) A one-page summary that shows how many employees and dependets are on each health plan, rx and dental with the annual costs for health, dental and vision used for budgeting.
