This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested: I am requesting a full and complete list of all NJ DOH OEMS 4-Digit NJ Paramedic (ALS) Certifications/Licenses Issued in the State of NJ since the inception of the Paramedic standard governed by N.J.A.C. 8:40, 8:41, and 8:41A.
This request includes all expired, re-issued/recycled numbers, associated with name(s) and date of initial issuance. No other demographic or geographic information is requested, nor is level of endorsement including Instructor or Flight status.
