Dear New Jersey Department of Community Affairs, Division of Fire Safety,
This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Please provide the applications with the supporting documentation (submitted by the applicant or on behalf of the applicant) for any certifications and/or licenses issued or denied for the following persons: James Bird II
This request shall include any documents submitted by the applicant or on behalf of the applicants and or any documentation or communications from the Divison of Fire Safety to the applicants listed above for the following certifications and/or licenses: Firefighter 1, Firefighter 2, Hazardous Materials Awareness, Operations, Technician & or Specialist, Hazmat Incident Commander, Incident Management Level 1, 2 & 3, Fire Officer 1, 2, 3 & 4, Fire Investigator, Fire Police Officer, Fire Inspector, Fire Official, Fire Instructor (Levels 1 and 2) and Drill Ground Instructor.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.
