Pursuant to the New Jersey Open Public Records Act (OPRA), N.J.S.A. 47:1A-1 et seq., I respectfully request copies of any and all incident reports, fire investigation reports, emergency service call logs, or other documents related to any fire, smoke, or hazardous material event at the property located at:

Please include any responsive records from January 1, 1968, to the present.

If records are not available electronically, I request scanned PDFs or paper copies. Please inform me of any associated fees before fulfilling the request.

This is a valid request under OPRA and the common law right of access. As per N.J.S.A. 47:1A-6(f), I am not required to complete a specific form to submit this request.

I have not been convicted of any indictable offense in New Jersey, any other state, or the U.S.

I will not use the requested records for a commercial purpose.

I am not seeking these records for a legal proceeding.
