Dear Custodian of Records:

Pursuant to the New Jersey Open Public Records Act (N.J.S.A. 47:1A-1 et seq.), please accept this as a request for access to government records held by the Passaic County Prosecutor’s Office regarding Jordan P. Kennedy, age 23, date of birth September 10, 2002, graduate of Rowan University.

I request copies of the following government records:

1. Employment Records

• Any record identifying his job title, hire date, position classification, division or unit assignment, length of service and current employment status.
• Any employment application, resume, or cover letter maintained as a government record.
• Any appointment documentation, personnel action forms (PAFs), probationary notices, or memoranda reflecting his employment.

2. Compensation & Pension-Related Records

• Any records disclosable under OPRA showing salary, step, contractual pay rate, and effective dates.
• Any records showing enrollment in PERS, PFRS, or any other NJ State pension system that are not exempt, including dates of enrollment and employing unit.
(I am not requesting SSNs, personal contact information, medical records, or any exempt identifiers.)

3. Police Training Commission (PTC) / Academy Records

• Any Notice of New Appointment (PTC-15 or equivalent) submitted to the New Jersey Police Training Commission for Jordan P. Kennedy.
• Any documentation submitted to the Division of Criminal Justice or any police training academy on his behalf, including:
– Pre-Service or Basic Course for Investigators applications,
– Enrollment or roster submissions,
– Orientation or intake forms,
– Background or eligibility verification forms.

4. Other Permissible Personnel Records

• Any oath-of-office card, oath form, or swearing-in certification.
• Any appointment resolution or certification showing governing-body approval, if applicable.
• Any publicly disclosable disciplinary records required to be released pursuant to N.J.S.A. 52:17B-247 (if applicable).
• Any organizational chart or staffing list showing his name and title.

5. Application Materials (OPRA & Executive Order 26)

Pursuant to N.J.S.A. 47:1A-1 et seq. and Executive Order 26 (McGreevey, 2002), please provide:

• Any résumé, cover letter, employment application, or similar application-packet materials submitted by Jordan P. Kennedy, to the extent such materials are disclosable for a successful candidate under Executive Order 26.
• Any supplemental written statements, questionnaires, or non-exempt materials contained in his application file that OPRA and EO 26 permit to be released.

For clarity:
I am not requesting materials of unsuccessful applicants, and I understand those are exempt under EO 26. I request only the application materials of Mr. Kennedy, if he is the successful appointee, as permitted by OPRA and Executive Order 26.

If any portion of this request is denied, please provide the specific statutory basis for the denial as required by N.J.S.A. 47:1A-6. If records exist electronically, I request them in electronic form via email.
