Employee Insurance Cost
Dear Paterson City.
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
Open Public Records Act (OPRA) Request
Pursuant to the New Jersey Open Public Records Act (OPRA), N.J.S.A. 47:1A-1 et seq., I request copies of the following existing government records relating to the municipality's employee health insurance program.
Unless otherwise specified, this request is limited to records for the current plan year and the two (2) immediately preceding plan years.
Please provide the following records, to the extent they are maintained by the municipality or maintained on the municipality's behalf by its insurance carrier, broker, or third-party administrator:
Monthly health insurance premium invoices, billing statements, and premium registers.
Summary of Benefits and Coverage (SBC) documents for each health plan offered to employees.
Monthly or annual claims reports provided to the municipality, including paid claims reports, utilization reports, large claimant reports, high-cost claimant reports, stop-loss reimbursement reports, and claims summaries by category (medical, pharmacy, behavioral health), if maintained.
Monthly enrollment reports or census reports reflecting covered employees and dependents by coverage tier or plan.
Insurance renewal proposals, renewal notices, renewal rate summaries, actuarial reports, underwriting reports, and rate justification documents received from the carrier, broker, or consultant.
Current and prior health insurance plan documents, Summary Plan Descriptions (SPDs), administrative services agreements, and third-party administrator agreements.
Stop-loss or reinsurance policies, declarations, schedules, attachment point summaries, premium invoices, and reimbursement reports.
Correspondence, including emails and letters, between the municipality and its health insurance carrier, broker, consultant, or third-party administrator concerning:
premium increases;
renewal negotiations;
plan design changes;
open enrollment; or
employee health insurance renewals.
Monthly or quarterly reports provided by the carrier or third-party administrator, including loss ratio reports, trend reports, utilization reports, and cost-sharing summaries.
Broker of Record letters, Producer of Record letters, consulting agreements, commission disclosures, compensation disclosures, and fee agreements relating to the municipality's employee health insurance program.
I request that responsive records be provided electronically in their native electronic format whenever available (including PDF, Excel, or CSV). If records exist only in paper format, please advise before duplication.
If any responsive record is withheld or redacted, please identify the specific statutory exemption relied upon and provide all reasonably segregable non-exempt portions of the record pursuant to N.J.S.A. 47:1A-5(g).
If fulfillment of this request will exceed $25.00 in fees, please provide a written estimate before processing.
This request seeks existing identifiable government records only and does not request that the municipality create new records, perform research, answer questions, or compile information.
I certify that I haven't been convicted of an indictable offense, will not be used for commercial purpose and this isn't in connection of a legal proceeding.
