Department
Job title or position
Salary or salary range/band
Hire date or years of service (if available)
(Please exclude names and replace any identifiers with anonymized or sequential IDs if necessary.)
Any existing aggregate reports or summaries related to employee health benefits, including:
Total number of employees enrolled in each health plan tier (e.g., single, family)
Total number of employees who have opted out of health benefits
Any available aggregate (non-individualized) annual cost or claims summaries for employee health plans
I am not requesting the creation of new records; only existing records responsive to the above.
