Emergency Response Records Request
Dear Highland Park Borough,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
For the property located at 220 South 4th Avenue, Highland Park, NJ 08904, for the discrete time period beginning January 1, 2019 through the date of fulfillment of this request, please provide:
1. All Computer-Aided Dispatch (CAD) records, dispatch logs, and call-for-service entries documenting any police, fire, or emergency medical response originating at, dispatched to, or initiated from the above address. Fields requested include but are not limited to: date, time received, time dispatched, time on scene, time cleared, call type / nature code, disposition, responding units, and incident/CAD number.
2. All Highland Park Police Department incident reports, case reports, supplements, and arrest reports associated with the above address.
3. All Highland Park Fire Department run sheets, NFIRS incident reports, and fire investigation reports associated with the above address.
4. Any EMS dispatch records, mutual aid logs, or first-aid squad call entries in the Borough's or Police Department's custody associated with the above address. I understand that patient care reports (PCRs) containing protected health information are exempt under HIPAA and N.J.S.A. 47:1A-1.1; PCRs are not being requested.
REDACTION REQUEST
I am requesting these records with all personally identifiable information redacted. Specifically, please redact:
- Names of all private individuals (residents, callers, complainants, victims, witnesses, suspects, and any juveniles)
- Dates of birth, Social Security numbers, driver's license numbers, and license plate numbers
- Personal telephone numbers and email addresses
- Any information protected by Daniel's Law (N.J.S.A. 47:1B-1 et seq.), HIPAA, the Crime Victim's Bill of Rights, or otherwise exempt under OPRA
I am interested in the occurrence of calls for service at the address — call type, date, time, disposition, and responding units — not in identifying any individuals involved. Names and identifiers of sworn officers and other public employees acting in their official capacity should remain unredacted to the extent permitted by law.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

file. Highland Park Police Officers are dispatched by Rutgers University
Police Department as
part of a shared-service agreement. As such, some records are maintained
by and must be
requested through Rutgers University.
