Dear Hillsborough Township,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:

Documents identifying any elected official who was covered by health insurance paid for (in whole or in part) by the Township of Hillsborough for any portion of the year, and for each such person, documents indicating the following:
1. The amount paid by the Township of Hillsborough for health insurance during that year, net of any contribution by the elected official;
2. The amount contributed by the elected official for health insurance during that year;
3. The type of coverage, e.g. individual, family, or couple;
4. The services covered, e.g. medical, prescription, dental, vision, etc.

Notes:

A. This request is for information about elected officials only.
B. This request seeks information solely about insurance coverage and does not seek health or medical information for any person.
