Please be advised that this firm has been retained by Citizens United Reciprocal Exchange (CURE) with regard to a motor vehicle accident which occurred on August 3, 2026 at
approximately 7:00 a.m. Wherein our insured’s vehicle was involved in a collision with a pedestrian (Case No.: 26036230). According to the police report (which can be provided upon request), the reporting officer was Police Officer Hana Lim, Badge #181. The dash cam footage from the truck behind the involved vehicle was submitted according to the police report. CURE respectfully requests that you maintain the video footage from the
dash cam and that same be provided to CURE.

Please advise if there is any cost associated with obtaining this video. Failure to preserve this evidence may result in a court action seeking damages. Please feel free to contact the undersigned should you have any questions.
