Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

Records requested:
Motor vehicle accident on the Garden State Parkway, just before State Route 3. We have the crash report number as D040202500396C for an accident from 06/01/2025, when searching on the reports site with the report number, name of driver and plate it states the report is not ready but it was from a year ago. I am with Helmsman Mgmt, his auto insurance and trying to determine who the other vehicles involved were in this impact as there was a accident just before our driver came on the scene and hit debris.

Driver Youliang Zeng, plate NY LVP1336, toyota sienna. Injuries are being claim by a passenger in the vehicle so need the report to locate the other vehicles.
