Dear Hackensack City,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Under OPRA and the common law right of access, I am requesting the following records regarding Hackensack Police Department Crash Report I-2025-009067, investigated by Officer Damian Colon on or about September 24, 2025, near 400 Myer Street, Hackensack, NJ:
1. A copy of the police crash report (NJTR-1) for that case.
2. A log, record, or list identifying all individuals, insurance companies, attorneys, or other entities who have requested, purchased, or obtained a copy of that crash report from the Hackensack Police Department or any authorized third-party vendor (e.g., CrashDocs, LexisNexis, Carfax, etc.) since the date of the crash.
3. Any record, correspondence, or receipt showing when and to whom the crash report was first released or transmitted.

My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

Please be advised that the Hackensack Police Department does not have an officer by the name "Damian Colon" nor does the Hackensack Police Department format their case numbers in the format that you provided below, the HPD formats their case numbers by the year and number of report - "25-xxxxx" not "I-2025-xxxxx". I have also double checked with the Hackensack Police Department, and they confirmed that they do not have an officer by that name nor do they have an accident report from that date and location with the provided case number. Kindly contact the department of where that officer is associated with in order to obtain the responsive records pertaining to your request.
