County EMS Program Profit/ Loss Statement
Dear Monmouth County,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
- Seeking to know what they have spent of ambulance and equipment for the ambulances and EMS program since the program inception.
- Cost of total expenditures related to disposable medical supplies since the EMS programs inception.
- Total cost of per diem and part time employee salaries for EMS personnel.
- Total cost of full time employee salaries for EMS personnel to include health insurance, dental, 401k, eye coverage, and pension costs since the programs inception for ALL Full timers.
- Total cost of management and supervisors as well as assistants and training personnel salaries overseeing the EMS program since inception.
- Total costs of uniforms for all EMS personnel since programs inceptions.
- How much has the county spent on ambulances, chase cars, supervisor vehicles, special operations vehicles, first responder vehicles, fly cars, etc for the County EMS program.
- Total cost of the county rehab truck purchased through OEM.
- How much is the profit vs. loss? Please attach a profit loss statement to this OPRA request.
How much money has been collected versus billed?
