Good Afternoon Mr. Taylor,
You didnt need to even file an OPRA to obtain this information and documents...my office is always here to help with election and petition filing :)
One item at a time:
Attached you will find the physical traditional nominating petition, which must be returned to my office no later than April 5th at 4:00PM. This would be utilized when obtaining live signatures. If you would like these forms mailed to you, or would like to schedule a pick up, let me know and we can work out the details.
I have also included the digital nominating petition for one candidate which can be completed and electronically submitted to me...If you have more than one candidate running, there are also digital petitions for one signer for two or three candidates. They can be found at the following link:

There are two female and two male Democratic County Committee seats that will be open.
The number of signatures needed for a democratic candidate is 10 signatures, a republican candidate needs 4 signatures. I always suggest that you obtain a few extra in the event that any of the signatures were to be challenged.
I have also attached the affadavit of circulator form. The oath of candidates is actually on the traditional nomination petition.... The affadavit form does need to be notarized.

If I am understanding correctly, using “Nominating PRIMARY Petition 2021” with all four candidates on one form, a Republican slate would require a minimum of four signatures and, a Democratic slate would require a minimum of 10 signatures, since people would be signing for all four candidates at once using that form. Is this correct or have I misunderstood?

Yes, You may list all 4 candidates on the same petition. There is additionally an affadavit for the petition circulator that I included. Whomever circulates the petition would complete this simply stating that they are verifying that the signatures are actually obtained from those individuals.
You are correct with the number of signatures required as well. HOWEVER, i do suggest that you obtain additional signatures in the event any are challenged. The signatory must be registered with the party that they are signing for (ie. a registered republican would not be a proper signer of a democratic candidate). They must be a registered voter.
Republicans and Democratic candidates cannot be on the same petition.
The only County Committee seats available this cycle are the democratic candidates.
Additionally there are two councilmember local seats to be elected.
