This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:
1. Time and attendance records for the past 3 years to include Vacation days, Sick Days Personal Time, Comp Time. Any time off for Workers Compensation claims or Suspensions.
2. Time clock records when he punched in for work and out of work. This is to include palm/fingerprint scanners for the past 3 years.
3. Copies of approval of Time records for Justin Lamb for the past 3 years.
4. Copies of Purchase orders and supporting documentation for any Training or classes attended and reimbursed to Justin Lamb. This is to include any College or Law School classes for the past 10 years.
