OPRA Request: Dean’s Natural Food Market
Dear Ms. Ippolito:
This Open Public Records Act (OPRA) request is respectfully submitted from the Monmouth County Regional Health Commission No.1 to obtain the following documents:
1. Copies of every Restaurant Inspection of the Dean’s Natural Food Market, located in Ocean Township, since the restaurant had been first inspected to present.
2. A copy of all documents delineating the results of the Monmouth County Regional Health Commission No.1’s January 16, 2018 inspection Dean’s Natural Food Market’s Ocean Township location, when the store’s restaurant was rated “Conditional”, as well as June 20, 2018 “Routine Re-Inspection” of January 30, 2018 when the store’s restaurant was rated “Satisfactory”.
3. Copies of any complaints made by one or more members of the public to the Monmouth County Regional Health Commission No.1 regarding Dean’s Natural Food Market in Ocean Township.
