5. Contractor phone number on file
6. Contractor email address on file
7. Property address
8. Work type/description

If an export from the SDL system in spreadsheet or CSV format is available, that is the preferred delivery method. Otherwise, any format containing the above fields is acceptable.

I certify that I have not been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

Please let me know if there are any fees associated with this request. I am willing to pay reasonable copying/processing costs.
