Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Records of employment for officer k. Jazikoff badge #-1216 with Neptune police department, copy of sworn oath of duty and responsibility to protect public, any licenses or certifications related to employment including any documents or prior complaints alleged or filed against same throughout entirety of employment with department as well as any disciplinary actions taken or listed on record please include any unfounded allegations as well and if details being specific aren’t allowed for release then overall total number of misconduct filings will suffice with corresponding date and any further details allowed and in accordance with opra laws , request for the name, rank and badge number of this officers immediate supervisor and finally, same for her superiors supervisor please include if employment history documents any record or history of complaints made for any of 3 requested individuals throughout Employment history with the department .

Any complaints made regarding any overall officer employed at or with Neptune police department or made about overall dept as a whole in last year including any findings or investigations regardless of outcome include any ongoing or present investigations into allegations of misconduct, suspicions of any alleged crimes related to performing traffic stops and searches and seizures of property .
Please include if possible officer k. Jazikoff number of traffic citations , tickets issued and warning given overall calculated on a monthly total basis for last 6 months. Please include number of arrests made monthly listing her as arresting officer and number of arrests pertaining to allegations involving use possession or sale or narcotics , amount of traffic stops where a search was conducted, stops that resulted in arrests overall and particularly arrests total monthly for narcotics or involving same in any capacity, if possible please include any charges that listed here as arresting or complaining officer pressing criminal charges on anyone over last 6 months including the charges that resulted in pleas, prosecution and those subsequently dropped or dismissed entirely by number not specifics or details overall categorized in monthly total basis for last 6 months. Please include on average overall for same officer highest average number of tickets she has issued throughout month highlighting particular week of month where most tickets were written or given on traffic stops. Any available employment records pertaining to this officer will be appreciated in accordance with and allowed by opra laws please.
