Complaint Investigation #C2025-7259 (Holmdel Township Public Schools)
Dear New Jersey Department of Education,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.

Dear Custodian of Records,
Pursuant to the Open Public Records Act, N.J.S.A. 47:1A-1 et seq., I request access to and copies of all records related to Complaint Investigation #C2025-7259, filed by Matthew and Kai Collins regarding Holmdel Township Public Schools.
This request includes, but is not limited to, the following categories of records:
1. All submissions in the complaint record:
• All documents, exhibits, and evidence submitted by both the complainants and Holmdel Township Public Schools.
• All correspondence between the parties and NJDOE related to this complaint.
2. Investigator and OSE records:
• Investigator notes, memoranda, working drafts, and internal analysis prepared during the investigation.
• Any supervisory review, quality control documents, or sign-off records related to this investigation.
3. DOE internal policies and guidance:
• Training manuals, rubrics, checklists, or internal guidance documents used by the Office of Special Education for complaint investigations.
• Any written DOE policies on how evidence is to be weighed or considered in state complaint investigations.
4. DOE–District communications:
• All communications (emails, letters, meeting notes, phone logs) between DOE staff and Holmdel Township Public Schools concerning this complaint.
5. Complaint tracking and oversight:
• Internal complaint tracking logs, databases, or spreadsheets showing how this complaint was coded, assigned, and closed.
• Any internal reports, memoranda, or compliance monitoring records prepared by DOE in connection with this complaint.
6. Other related complaints and oversight:
• Any other state complaints filed against Holmdel Township Public Schools within the past five years, including findings, corrective actions, and DOE internal notes.
• Any OSEP (U.S. Department of Education) correspondence, monitoring reports, or audits referencing DOE’s handling of complaint investigations or Holmdel Township Public Schools.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

Dear Requestor,
The Department of Education is in receipt of the above referenced OPRA
request. It appears that you may be the parent of the student for whom you
are seeking the protected education records. Under FERPA, a parent can
request access to their child’s education records. I have attached an
authorization form that will allow me to provide these records to you
unredacted. Please fill in all information in the highlighted sections,
and sign and date the form. Upon receiving the completed form, I can
release the unredacted records to you.
Additionally, pursuant to N.J.S.A. 47:1A-5(g), requests for correspondence
must contain: (1) the name or specific job title/position of the sender
and/or recipient, (2) a specific subject matter, and (3) a specific date
or range of dates, which must be confined to a reasonable time period.
Please provide this information so that we may process this part of your
request.
Furthermore, please provide an alternate email address that we can use to
provide these records to you, as we cannot provide unredacted student
records through the OPRA Machine that you used to submit your request.

I received the below email below on Friday, October 3, 2025, but I didn't see an email from MoveIt Cloud. Please send any links to [email address]. This is my alternate email that I need to utilize for my OPRA requests.
"Due to the size of the files, which exceed the Department of Education’s email system limitations, records responsive to the above referenced requests are being provided through the MoveIT cloud file sharing system. Shortly, you should be receiving an email from MoveIT Cloud with instructions on how to access your records. If you cannot download the records for any reason, please let me know by replying to this message. Please note, the contents of the MoveIT Cloud email will expire in 15 days. Additional time is needed to determine whether the Department of Education maintains additional records responsive to your requests, and to review and process any additional records that may exist. Accordingly, the due date is extended to October 14, 2025. A receipt indicating same is attached."
