All of the below listed requested records are in reference to Antonio Manata of 1 Crescent Parkway Clark NJ 07066 during his employment with the Clark Township (Union County), from July 2007 to present only. “NJ CSC” refers to the New Jersey Civil Service Commission.

- Current Status/standing of Antonio Manata with the NJ CSC,
- All “Final Notice of Disciplinary Action 31-B” (FNDA 31-B) forms submitted to the NJ CSC by the Clark Township
- Records showing changes of status/rank/position and respective dates of Antonio Manata
- Communication (email/fax/regular mail/database) from the Township of Clark requesting the certification of all promotional lists Antonio Manata was on, to include dates of said communication and rank being certified.
- Communication (email/fax/regularmail/database) from Township of Clark advsing NJ CSC about changes of rank/position for Antonio Manata to include dates and rank change,
- Communication (email/fax/regular mail/database) between the NJ CSC and the NJ Division of Pensions and Benefits since 1/1/21 in reference to Antonio Manata
