Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

insufficient):
1. The current Certificate of Occupancy (CO) for the property and for the second-floor unit
specifically.
2. The current Certificate of Code Compliance and/or Certificate of Habitability issued for any rental
unit at this property.
3. The landlord/rental registration statement on file for this property, including the name and
address of the record owner and any managing agent.
4. The property's legal use classification and the number of dwelling units the property is legally
approved to contain.
5. The maximum number of occupants/tenants approved for the second-floor unit.
6. Any records, permits, or approvals authorizing the rental of individual rooms (room-by-room or
'rooming house' occupancy) at this property.
7. Any open or closed code violations, complaints, summonses, or enforcement actions associated
with this property from January 2025 to the present.
8. Any inspection reports for this property from January 2025 to the present.
