This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:

All records related to city sewer system failures for the past 10 years including those requiring pumping service from the town and resulting in damage to private property due to sewerage back up.

If the town does not have these records, then provide contact information for the local authority responsible for providing these records.
