This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:

Please provide a copy of the cancelled checks and remittance details for all health, prescription, and dental insurance paid from January 2017 to the most recent payment.
Please provide a copy of the cancelled checks and remittance details paid to the various brokers for their services during this same time period.
