Dear New Jersey Department of Community Affairs, Division of Fire Safety,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

Records requested:
Please provide the applications with the supporting documentation (submitted by the applicant or on behalf of the applicant) for any certifications and/or licenses issued or denied for the following persons: David Terzian, Anthony Mangone, Brian Doel, Gregory Griffis, and Edwin Arevalo. This request shall include any documents submitted by the applicant or on behalf of the applicants and or any documentation or communications from the Divison of Fire Safety to the applicants listed above for the following certifications and/or licenses: Firefighter 1, Firefighter 2, Hazardous Materials Awareness, Operations, Technician & or Specialist, Incident Management Level 1, 2 & 3, Fire Officer, Fire Investigator, Fire Police Officer, Fire Inspector, Fire Official, Fire Instructor (Levels 1 and 2) and Drill Ground Instructor.

Attention Anonymous OPRA Requestor:

 

The New Jersey Division of Fire Safety (“Division”) is in receipt of your
public records request wherein you seek “the applications with the
supporting documentation (submitted by the applicant or on behalf of the
applicant) for any certifications and/or licenses issued or denied for the
following persons: David Terzian, Anthony Mangone, Brian Doel, Gregory
Griffis, and Edwin Arevalo. This request shall include any documents
submitted by the applicant or on behalf of the applicants and or any
documentation or communications from the Divison [sic] of Fire Safety to
the applicants listed above for the following certifications and/or
licenses: Firefighter 1, Firefighter 2, Hazardous Materials Awareness,
Operations, Technician & or Specialist, Incident Management Level 1, 2 &
3, Fire Officer, Fire Investigator, Fire Police Officer, Fire Inspector,
Fire Official, Fire Instructor (Levels 1 and 2) and Drill Ground
Instructor.”

 

The Division has reviewed your request and is forwarding copies of all
documents in its possession responsive to your request.  Please note that
Applications, Supporting Documentation and Division internal computer
records are attached for Brian R. Doel, Edwin R. Arevalo, and David
Terzian.  As for both Anthony Mangone and Gregory Griffis, the Division
has no Applications or Supporting Documents in its possession, and
accordingly, is providing merely Division internal computer records.

 

Please be advised that on all relevant records, redactions have been made
for personal identifying information (i.e., Social Security Number, Home
Address, Home/Cell Numbers, Birthdate, personal email address and Drivers
License Number where applicable). 

 

Upon transmittal of these records, the Division will deem your public
records request satisfied and closed.
