For the information you are requesting please complete the online OPRA request application at following the information below.
Please click on the link provided to access the Division of Alcoholic Beverage Control : https://www.njoag.gov/about/divisions-an... scroll down to the lower left side and under CONTACT click on PUBLIC RECORDS REQUEST. In the middle of the next page you will click on Request Forms which will provide you the Online Request Form click on that. You will then have to Choose a Department/County, you will select Law and Public Safety which will bring you to again Choose an agency/division, you will select Division of Alcoholic Beverage Control and fill in the form provided in all of the blue highlighted areas and click on submit when completed.
