Dear Jersey City,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States that would disqualify me from this request.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a scheduled or open legal proceeding.

Records requested:
There are records involving JCPD for January 12th 2023 & EMS that are not in JCPDs current reports on this incident. I want them also.
I am seeking the 911 phone call and the call made to Mobile Crisis before the screener Genesis Rios was sent to make contact on site. 1/11/2023.
I am seeking a transcript of the call made while JCPD was inside my home before Genesis Rios ever saw/spoke to me inside 88 South Street. Genesis Rios and the name of the Mobile Crisis member that was communicating with the 911 operator. 1/11/2023
I am seeking all unedited body worn camera footage due to the high number of responding officers, there may be vital information going on out of the focus of the lens that only I would find important in proving a crime was occurring at that incident.

I am seeking further information JCPD has on the incident associated with report #'s
23-007287 Officers DeLeon and Morante were not only Officers on site. ESU Villa Nueva and I interacted. I remember a female partner for DeLeon. BWC footage unedited.
24-020350 is CAD incident number and the department case number is 24-019419
Shield #2864 & #2721were the responding officers named in above request

My preferred delivery method for response(s) to this request is by USPS first class mail with the OPRA account login information to minimize its interception. If there is an office within 20 miles of my home I would gladly pick up the records in person. I will pay whatever fee is fair and just or make a donation to help the OPRA system maintain its services, Please confirm you have received this request.
