Borough of South River EMS BLS Provider Application

These requests are in regards to the BLS Provider application submitted by the Borough of South River for the Borough of South River EMS.
I requesting a copy of the agency's SOP, a copy of the staff roster and a copy of the provider and vehicle survey submitted by the Borough of South River in their application/follow up to the application to become a BLS Provider. I am requesting a copy of any paperwork showing the final approval or denial of the application, or any correspondence in regards to the application sent to or from the Borough of South River. This includes requests for additional information or extensions for the application process.

You provided the information for the wrong agency. You provided information for South River EMS, Inc. The request was for the Borough of South River EMS. The Borough of South River submitted a BLS Provider application in December 2018 with the agency named "The Borough of South River EMS". The paperwork requested would have been submitted as part of the application.

The application was not completed. The Borough of South River is not a licensed provider. It is the Departments understanding that the service has been contracted to Robert Wood Johnson University Hospital Emergency Medical Services.
