Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

I have been displaced due to an illegal eviction and lock out where my belongings still reside with Jeremiah Thomas who also committed what I beleive also was rental fraud as per other Opra requests submitted. I am unable to go to the City Hall due to this displacement as I am currently located now in Pinellas Park FL. Where I had a living space available for me.

I am requesting bodycam footage related to a call made on Nov 8th to the 5th presinct related to 112-114 Maple Ave. Around 11:30am-12:30pm. My name is Michael Ware and I am requesting this footage as the officer did not take my complaint of assult seriously. I did not receive a case number. Jeremiah Thomas the individual who assaulted me made false statements to the officer which I would like to include in my evidence to the detective
