Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

Pursuant to the New Jersey Open Public Records Act (OPRA), N.J.S.A. 47:1A-1 et seq., I am requesting access to and copies of all government records related to the death of Anthony Rogers, age 54, of Jackson, New Jersey, DOB 05/23/1968, who passed away while at a friend’s residence located at 4504 Norma Place, Toms River, NJ.

Specifically, I am requesting the following records for this incident, including but not limited to:

All body-worn camera (bodycam) footage

All dash camera footage

All 911 call recordings and dispatch audio

All incident reports, investigation reports, arrest reports, supplemental reports, and narratives

Any CAD (Computer-Aided Dispatch) logs

Any use-of-force reports, if applicable

Any medical response or EMS-related reports in possession of your agency

Any photos, videos, or audio recordings related to this incident

Any communications or records documenting the response to the incident

If any portion of this request is denied, please provide the specific statutory exemption relied upon for each denial, as required by OPRA. If records are partially exempt, I request that any non-exempt portions be released with redactions where necessary.

I request that records be provided in electronic format where available. If there are any fees associated with fulfilling this request, please inform me prior to processing.

If this request is overly broad or requires clarification, please advise so that it may be narrowed appropriately. If the requested records are maintained by another agency, please forward this request or advise where it should be directed.
