We are in receipt of your OPRA request.  Due to new legislation, the State
and all municipalities have adopted a new official form which includes
additional statements above the signature line that need to be answered
for a request to be deemed complete.    

 

Even requestors that choose not to use a form must within their written
request include items in the newly adopted form that includes these
additional questions per NJSA 2C:28-3.

 

I have attached a copy of the form for reference, please respond including
the three completed statements, failure to do so will result in the
request being denied due to the request being incomplete.
