As advised in my September 11th response, a copy of which is attached, OPRA reform legislation that went into effect on September 3rd requires that either the attached new form be submitted for all requests or that you provide, in writing, the required information regarding whether the records will be used for a commercial purpose and whether they are being sought in connection with a legal proceeding. At this point in time, your request is still considered incomplete and will only be processed upon submission of the appropriate form or furnishing of all the required information.
