Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Pursuant to the New Jersey Open Public Records Act (OPRA) N.J.S.A. 47:1A-1et seq., I request copies of the following government records relating to the Request for Proposal (RFP) for the "Youth Empowerment Support (YES) Teams" grant/program issued during January 2023 through July 2026:

1. The complete RFP package, including all drafts, final versions, attachments, exhibits, specifications, evaluation criteria, and scoring methodology.
2. All bid or proposal submission received in response to the RFP.
3. All bid tabulations, evaluation score sheets, scoring summaries, evaluator comments, ranking sheets, interview score sheets, and final recommendation memoranda for each proposer.
4. Records identifying each member of the evaluation committee, including appointment memoranda or documents establishing committee membership.
5. Any records reflecting changes or revisions to proposal scores, rankings, interview eligibility, or recommendations after the initial evaluation.
6. All communications, including emails, letters, memoranda, meeting minutes, text messages maintaining by the agency and attachments, between any agency employee or official and any consultant or consulting firm recommended to Monmouth County in connection with the proposal or grant application.
7. Any conflict-of-interest disclosures, recusals, ethics forms, or certification committee members relating to the procurement.
8. The executed contract(s), purchase order(s), notice(s) of award, and any amendments resulting from RFP.
9. Any protest, appeal, challenge, or correspondence concerning the evaluation, scoring, interview selection, or award.
10. Any documents reflecting the basis for determining which applicants advanced to interviews, including records showing whether any applicant's score was changed or reconsidered after the initial evaluation.
11. Any records reflecting communications with the grant funding agency regarding proposal scoring, interviews, or award recommendations.
