Dear Boro Clerk,

I was unaware that the template which is widely circulated on various freedom of information online forums and which I copy and pasted from such a source mentioned ambulance reports.

I am unwilling to have any contact with the individual named in the subject line of this request, knowing what he is convicted of. I will therefore not be obtaining any medical release or similarly worded forms from him with his signature.

I am willing to accept the rest of my request, without ambulance or municipal court records.
