Dear Mountain Lakes Borough,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:
Pursuant to the New Jersey Open Public Records Act, please provide access to and copies of the following specific public records regarding the selection, procurement, and hiring of Appraisal Systems, Inc. (ASI) for the municipal property revaluation:

The final executed contract and addenda, , between the Township/Borough and Appraisal Systems, Inc. (ASI) for property revaluation services.

The complete Request for Proposals (RFP) or Request for Qualifications (RFQ) issued by the municipality for the revaluation project, including the specified selection criteria and a description of the selection process.

All bids, proposals, qualifications, and response packages submitted to the municipality by Appraisal Systems, Inc. (ASI) in response to the aforementioned RFP/RFQ.

All public board, council, or committee resolutions authorizing the advertisement of the RFP/RFQ, the acceptance of ASI’s bid, and the execution of the final contract with ASI.

The final evaluation scoresheets, ranking matrix, or written selection reports generated by the municipality’s evaluation committee, business administrator, or tax assessor during the review and selection process of the bids.

My preferred delivery method for response(s) to this request is by E-mail as attachments.
