and either fill out the new form or answer the following questions:
1. I have or have not been convicted of any indictable offense under the
laws of New Jersey, any other state, or the United States.
2. I, or another person will or will not use the requested government
records for a commercial purpose
1. I am or am not seeking records in connection with a legal proceeding
If confirmed that the records sought are in connection with a legal
proceeding, please advise of that proceeding. Once completed the OPRA
will be processed.
