Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I respectfully request access to and copies of public records related to a reported incident involving a possible deceased individual at or near 1913 Atlantic Avenue, Manasquan, NJ 08736, occurring at approximately 6:00 a.m. (approximate time) on the date 02/10/2026 of the incident.

Specifically, I am requesting the following records:
*Any and all 911 call logs related to this incident.
*Audio recordings of all 911 calls associated with this incident.
*All Computer-Aided Dispatch (CAD) logs related to this call or incident.
*Any call detail reports, incident notes, narrative reports, or dispatch notes generated in connection with this event.

I understand that certain information may be exempt from disclosure. I am requesting that any exempt information be redacted as required by law, and that all reasonably segregable non-exempt portions of the records be released.
If any portion of this request is denied, please provide the specific legal basis for the denial as required under OPRA.

My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.
