Dear New Jersey Department of Corrections,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

1. All incident reports, police reports, accident reports, investigation reports, offense reports, complaint reports, arrest reports, and field reports.
2. All supplemental reports, continuation reports, investigative reports, follow-up reports, detective reports, and supervisory reviews.
3. All written statements, sworn statements, witness statements, victim statements, suspect statements, and any notes or memoranda reflecting oral statements.
4. All recorded statements, whether audio or video recorded, transcribed or untranscribed.
5. All police officer notes, field notes, notebook entries, memorandum books, handwritten notes, contemporaneous notes, and draft reports.
6. All photographs, digital images, photographic arrays, photographic evidence, and crime scene photographs.
7. All video recordings, including but not limited to:
o Body-worn camera recordings;
o Dashboard camera recordings;
o Mobile video recorder recordings;
o Station-house recordings;
o Interview room recordings;
o Booking area recordings;
o Surveillance recordings obtained or reviewed during the investigation.
8. All audio recordings, including but not limited to:
o Radio transmissions;
o 9-1-1 recordings;
o Dispatch recordings;
o Telephone recordings;
o Interview recordings;
o Recorded witness, victim, or suspect statements.
9. All CAD reports, dispatch records, event chronology logs, communications logs, radio logs, and call detail records.
10. All arrest-related documents, including:
o Complaints;
o Warrants;
o Affidavits;
o Probable cause statements;
o Booking records;
o Fingerprint records;
o Custody records.
11. All investigative materials maintained by the police department, prosecutor's office, or investigating agency relating to the referenced matter.
12. All evidence inventories, evidence logs, chain-of-custody records, and property reports.
13. All documents reflecting the disposition of charges, including amended complaints, dismissals, plea agreements, judgments of conviction, and sentencing records.
14. Any electronically stored information maintained in digital evidence management systems, including Axon Evidence, Evidence.com, WatchGuard, or similar systems.
15. Any records identifying the existence, retention, destruction, transfer, or unavailability of any responsive records.
