The New Jersey Department of Health (NJDOH), Division of Public Health
Infrastructure, Laboratories and Emergency Preparedness (PHILEP) received

address, phone number, and home address. If any of these items are
unavailable, kindly fill the remaining available items anyway.

The list can be provided electronically in any format available, excel or
CSV preferred.

Available records responsive to the portion of this request seeking the
names of active EMTs or EMT-Bs can be accessed by logging in to the Public
Records Center at the following link. In accordance with N.J.S.A. 47:1A-1
et seq. and Burnett v. County of Bergen, 198 N.J. 408 (2009), residential
addresses, phone numbers, and email addresses have not been provided,
as personal identifying information and/or information in which an
individual has a reasonable expectation of privacy. Please also note that
information for "home county" is not.
