I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.

Please provide separate responses to each of my three OPRA requests. My understanding is that, under the current OPRA law, these are considered three separate requests rather than a single request. Accordingly, I ask that each request be processed and responded to individually.

Dear Mark Spivey,
I submitted an OPRA request to the MCPO on May 9, 2018, using the same exact language as my current request. In response to that request, the MCPO provided the requested records.
Given that my current request uses the same exact language, I'm unclear as to why it is being treated differently. For your reference, I have included a link to my May 9, 2018 OPRA request and the MCPO's response, which demonstrates the records that were provided at the time.
As I did in 2018, I'm more than willing to grant the MCPO a reasonable extension to allow your agency additional time to obtain the documents responsive to my current OPRA request.
