Per Request #R032862-070126 submitted on 7/1/26 which should not have been closed, there are still an immense amount of documents per my last request still missing including permit application packages, invoices, tech cards, etc, all certs of approval and occupancies. This property is an assisted living congregate care facility regulated by DOH and DCA. I requested complete planning, zoning, and construction application approval packages. There should be agendas, resolutions, minutes, all building UCC forms, leases, ownership disclosures, subdivisions and site plans, waivers, All Submission Materials Including, Application Forms, Ownership Disclosure Statements, Certifications, Plan Sets, Zoning Permit Letters, Completed Checklist Forms, Written Request for Waivers with Description of Each, Listing of Property Owners within 200 feet. All Plat Information, Filing of Applications, Phasing Plans, Tax Lot and Block Numbers, All Plan Details with general Information including the Site Plan Maps with index sheets, Key & Zoning Map, Property Owners within 200 ft. Owner/Applicant Info, Use Restrictions, etc. Cover sheet, existing conditions, dimensions, grading, utilities, landscape plan and details, lighting plan, soil erosion & sediment control plan & details, Truck circulation plan, profiles, construction details All Supplementary Documents Including: Lists of all federal, state, regional, and/or muni approvals or permits required, Deed Restrictions and Covenants, Letters of Interpretation, Statements, Reports, Affidavits etc.

This would include: Permit application packages & Inspections: #05-0134, #09-0020, #09-3200, #11-0231, 11-3870, #12-03984, 12-02978+B, 15-06461, 16-01561, 16-01828, 17-01052, Fire: Pre Plan Info, Registrations, and Inspections Including Spring Oak Assisted Living, Local Id#4237. I am also looking for records of tax payments, any agreements, and ownership disclosures with development applications.
