2025 IA Summary and Major Discipline Report (Departments in Middlesex County)

The request was successful.

Dear South Plainfield Police Department,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:
1) Police Department record known as the "Internal Affairs Summary" (a/k/a Appendix K) for the reporting year 2025
2) Police Department record known as the "Annual Major Discipline Reporting Form" (a/k/a Appendix L) for the reporting year 2025

My preferred delivery method for response(s) to this request is by E-mail as attachments. However, you may also direct me to the precise location on your agency's website where these records are available.

Please confirm you have received this request. Thank you kindly.

Yours faithfully,

Charlie Kratovil

Sir,
I have received your email and I have attached the 2025 Annual Major
Discipline Reporting Form(appendix L).  While preparing the Internal
Affairs Summary(Appendix K), it was discovered that there was an error
with the report that we filed with the state and the numbers do not
accurately reflect the actual data; I believe this was due to formatting
from one system to another, but I am not sure.  I brought that information
to my higher ups and they are addressing that form.  I will advise if/when
I hear back from them with the proper data.
Respectfully,
Lieutenant Lloyd McNelly
