Records Access Officer: Pursuant to the New Jersey Open Public Records Act (“OPRA”), New Jersey S.A. 47:1A-13, I hereby request the following records be produced, in the original or common electronic format as described below:

The following formats are acceptable i. e.g. .csv, .xls, .xlsx, etc ii. For records that only exist on paper, Adobe PDF (scan) is acceptable. For records that exist only in electronic format, exact copies are requested to extent they are not in proprietary formats, etc. For records that exist only in electronic format, but contain sensitive information explicitly exempt from disclosure (such as a SSN), exact copies are requested to the extent they are not in proprietary formats, etc., with only the protected information (but not blank or null data) overwritten with some notation of the statutory exemption. If no records exist responsive to the above request(s), I hereby request written reply to that effect. For records produced in response to the request(s), I hereby request certifications that the record(s) produced are correct and to be emailed to me at [email address].

If my request does not adequately describe the records to be produced, please contact me via email so that I may clarify my request, and when appropriate inform me of the manner in which records are filed, retrieved or generated. Pursuant to OPRA, within five business days of the receipt of this request, I ask the courtesy of your reply or denial in writing, to include a reason of any denial in whole or part, or provide a date certain when such a response can be reasonably expected.
