Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

2. Incident/Investigation Reports – any police reports, investigation summaries, or narratives tied to incidents at this address or involving the named individual.
3. Body-Worn Camera (BWC) Footage – any and all available footage from officers responding to incidents or calls at this address or involving the named individual.
4. Arrest Records – documentation of any arrests made at, or involving, this address or the named individual.
5. Citations/Summonses – copies of any tickets, municipal complaints, or summonses issued at this address or to the named individual.
6. Noise Complaints – all reports and records of noise complaints connected to this address or involving the named individual.
7. Neighbor Disputes / Altercations – all reports, citations, or investigations involving disputes or altercations between residents of this address (or the named individual) and neighbors.
8. Other Responsive Records – any additional records of police interactions, reports, field interview cards, supplemental narratives, or other documents relating to this address or the named individual that are subject to disclosure under OPRA.
For each record, please include all attachments, supplements, and officer narratives. If any portion of this request is denied, please identify the specific statutory basis for the denial. If redactions are necessary, please provide the redacted records rather than withholding them in full.
