Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

1. The landlord registration statement currently on file for the Property, and all amended registration statements filed from January 1, 2023 to present.
2. All rent registration statements, registered legal rent records, and rent control/rent leveling records on file for the Property, without date limitation.
3. All applications for rent increases, hardship increases, capital improvement surcharges, or vacancy decontrol filed for the Property, and all determinations or decisions issued in response, from January 1, 2018 to present.
4. All property maintenance, housing, and construction code inspection reports for the Property from July 1, 2023 to present.
5. All notices of violation, abatement orders, summonses, and municipal court complaints issued to the owner or managing agent of the Property from July 1, 2023 to present.
6. All certificates of occupancy and certificates of continued occupancy issued for the Property from January 1, 2023 to present.
7. All complaints received by the City concerning conditions at the Property from July 1, 2023 to present. Personal identifying information of complainants may be redacted.
8. All correspondence between the Office of the Mayor, the Department of Public Safety/Code Enforcement, or the Rent Leveling Board and the owner or managing agent of the Property, concerning conditions, violations, or management of the Property, from January 1, 2023 to present
